Well, there are several reasons. One of which is that there are numerous narcotics that are just better at pain management.
Another reason is it causes a toxic metabolite, normeperidine, to accumulate with high doses and/or extended use, or in patients with renal insufficiency. Normeperidine can lead to CNS (central nervous system) excitation and seizures. Basically, the cons out- weigh the pros on this drug.

Not sure what you mean by "out of fashion" but the most commonly prescribed form of demerol now is Meperizine/Meperagan which is a mixture of demerol and phenergan (for nausea).

Of course it doesnt help the case that Lortab/Vicodin is the most commonly prescribed pain medicine today.

I didn't know that it was. Are you sure they are not using it as much as always in the hospital? If they aren't, one reason could be because newer drugs are constantly being tried that have less side effects. And Doctors are usually conservative. If pain can be effectively controlled with drugs a bit weaker than demerol, they will use that. Oxycodone (percodan), now in the form of oxycontin is used quite a bit.

It is a bad drug. It was derived from atropine and therefore has mild anticholinergic properties (increased heart rate etc.). It also has toxic metabolite that can cause seizures (due to its neurotoxicity), particularly with people who have kidney problems. It is good for one thing though. That is post op rigors (shaking after coming out of surgery). For some reason it works well for this other than for its pain fighting properties.
