She's like 6 1/2 years old. She was fixed when she was about 3 months old. Every now and then, she gets super affectionate, not a full "heat," but almost like a "mini-heat" with her butt in the air and such. Could this be left over hormones because she was so tiny when she got fixed?

It all depends on which method ur vet used to de sex the cat. Some vets just tie the tubes, some do a full hysterectomy. She isnt calling, so don't worry.
By the way 3 months isnt really tiny .... these days more and more vets are desexing at 8 weeks.

If your concerned see a vet... but for what is worth my male acts the same way when he is in an affectionate mood so it could just be a regular cat behaviour that singles the need/want of attention and have nothing to do with heat/mating

Not nessicarily left over hormones because they dont remove them, they just tie her tubes like they do with humans. Alot of cats do it, not all, but alot of them. Basically her body is telling her "WHAT?! You haven't mated? What kind of cat are you?!" It's nothing to worry about obviously since she isn't pregnant. She's just going through her motherly phases and her biological clock doesn't like her.

If you're cat is acting like she is in heat, whoever spayed her did not do it correctly. They left her ovaries in, which of course, trigger her behavior.

Take her to another vet to complete the surgery.
