I don't know if there is more than one mechanism, but one way a hot spring forms is when an earthquake forms a fault or series of faults that capture an aquifer. Then, water to seeps down through the fault to a depth deep enough to cause the water to turn to steam. The steam builds up tremendous force and it eventually finds a path to the surface through faults, fissures and small cracks in the bedrock where it emerges as hot water and/or steam.

Hot springs form wherever underground aquifers meet underground heat, usually from a volcano. Yellowstone Park is the best example - it's actually a massive supervolcano with springs all over.
