has anyone ever had any accidents happen in an office before?

please tell me a story about any accidents that have happened in your workplace. its for a school assignment.

I was taking some banding material off this big package. It was loose and as I walked around I tripped on it, and fell FLAT on my face. Thank goodness, no one else was around as I would have been really rediculed.
