Use cover-up in her skin tone and finish with translucent powder. Line the eyes in black and color in or put mascara on the eyebrows. Eyeshadow should be in browns or mauves, no greens or blues! Apply a darker shade to the eyecrease and extend beyond the outer egde of the eye. Using the same shade draw from the outer tip of the eye to the line you just made. Highlight the eye with lighter shades from the same palette. Remember to use blush and a bright lipstick.
When applying make-up for the stage - remember that the more the better! You make think like you look over done, but with all the lights on the stage you need the make-up to be exaggerated. This prevents the "sick look" on stage.

No matter the age:

mascara
blush- on cheeks and along hair line
marroonish red lipstick
dark brown eye shadow in the crease of the eyelid
light light brown to the eyebrow
eyeliner under the eye, extending slightly past
highlighter under the eyebrow, in the corners of the eye

For a five year old, this should be plenty!!

1. very vibrant blush on both sides of the cheek.
2. this is important: dark eyeshadow, because it's very hard to see makeup on stage.

Optional:

3. glitter right under the eyes so that the light catches on her face and gives it a little twinkle.

Just a little blush, not to much, and some lip balm or very little gloss-not to glossy. At least that's what they did for me same age and it looked pretty good.

Let her dancing be be what stands out on stage.

As far as I'm concerned, a 5-year old NEVER needs makeup.

I tried to add my suggestions about how to do it, but I just can't bring myself to recommend ANY makeup for such a young person. Really...can we say, "JonBenet Ramsey"?

I took ballet classes, and I know there was never any need for makeup when I was dancing.
