1040 long form. You will have to use an itimized deduction list instead of 1040EZ or 1040A.

Schedule A. This form can also be used for medical expenses (but you have to have a lot of those to make it worthwhile) and personal property taxes, such as cars, if your state levies a tax partly or wholly based on the value of the property. There are several other expenses you put on this form but they are not applicable to most taxpayers.
