For example you see someone attractive at the mall, school, etc, but she is a total stranger to you. how would you ask for a date?

You won't have the confidence to pull out conversations with a beautiful girl in the mall if you can't pull off conversations with normal strangers. Settle that issue of confidence first. (such a long issue to discuss here.. :| )
Make sure they're comfortable, through an interesting/engaging/fun/teasing conversation, take some time, and invite them to walk together or go for tea/coffee. don't forget to take their # later. :)

If there's a coffee shop nearby, ask her if she'd like to have coffee with you. If she says "But I don't know you", you can say "Well that will change once we talk over a cup of coffee" and smile.
Coffee is a lot less intimidating than dinner or a movie. Always ask coffee first
