All taxpayers file a Form 1040, you can get them at the post office or at http://irs.gov it will not be sent to you. Your employer will send you a form W2, you will use the information on it to fill out the form 1040

If you filed a 1040 last year, you will be sent one again this year. If you didn't get one and you need one you can go to any post office and get one or print one off from the irs.gov website
