I primarily count calories (intake and burned), but I also check to make sure that less than 30% come from fat, and that I'm eating my proteins and carbs in balance (15% protein or more, 55% carbs). Also count fiber (25-35g per day), calcium, and iron.

Sounds daunting, but there's an online program that I use to keep track of this for me. Lost 20+ pounds this way, have kept it off for almost a year, and definately eating healthy now.

Only out of interest. I sometimes do a lot of physical work and exercise and it's useful to know that I'm taking in 5 to 6,000 calories a day. Last week I was using up to 10,000 calories a day and needed to eat large amounts of high calorie food so I kept an eye on what I was eating.

Yes - I have been watching the calories for about a year and I dropped over 50 pounds.

Not really. I have a rough idea of how many calories are in most of what I eat and I try not to go overboard but not at the expense of my chosen lifestyle.
