No, theyre not aware of it. Unless. if they seek professional help, a psychologist may tell a person hes paranoid or something. Much like an obssessive-compulsive person. If he seeks for professional help, he'll know.

Some do - it depends on the degree of the illness, and whether or not they are getting treatment or being medicated. Some paranoia is actually reasonable in this day and age - its when it starts to affect your life negatively because you might be taking it to far, or you truly do have a brain chemistry disorder that is the biological cause of the illness. I know people with this condition, and with treatment and medication they can live relatively normal lives.

I believe it's certainly possible at an early stage, when the delusions are developing. I'll admit that I sometimes experience moments of persecutory delusion (and it does run in the family FWIW). The thought and awareness of this as a whole can be very disconcerting for me at times.
Oh and BTW I'd appreciate it if you'd quit watching me through my monitor... hehehe

I am bipolar and paranoia comes with my manic states. My friends and family recognize my symptoms. When they tell me, I am able to realize that I am paranoid, but the knowledge doesn't make it go away, which is very frustrating.
