To answer a part of your question about name recollection, and since I have the same problem too, (it's not out of the normal by the way) this is what you should do..
If you see the person you know just act normal, and to avoid the embarrassment of asking them "what was your name again?" Just ask them how to spell it, unless their names were short, LoL I can’t help you there!
Tip: the way I improve my memory is to abbreviate, let’s say I have 10 friends, and I need to remember all their names for a party or something, I would abbreviate their names. For instance: Jessica Simpson, is JS, Jennifer Lopez, JLo, Brad Pitt, BP, etc..
With phone numbers, let's say it were 0017345698, split it up in to 3 parts, thus 001 7345 698, get the idea?
Repeat, repeat, and more repetition! I can’t stress it more!
You can always resort to singing, if you have too ;)

Well, the brain is amazing and strange . . . some people can remember the strangest details but can't remember where the car keys are (in their pocket).
For some simple exercises to keep your mind going and may aide in memory:
Crosswords, number puzzles, card games.
Also try 'word of the day', take it in in the morning and try to use the word sometime during the day.
With phone numbers, try to see patterns in the numbers (use the key pad to see squares, triangles, etc.). Patterns are usually the easiest way to remember things. Good luck!

Try to associate the things you are trying to remember with other things, whether it be people, events, whatver you can do to help your brain have more to grab ahold to and recall later. This is why songs that teach you things work well, conjunction junction whats your function. Also wordplay ie HOMES for the Great Lakes Huron, Ontario, Michigan, Erie and Superior.

Just start to learn you are going to have to write things down, my memory is going cause of a medication i take, so i write stuff down now that i want to remember
You are getting older and your brain can only hold so much memory
Also you are stressing out so much that you want to remember stuff, so you cant

I read somewhere that your brain is a muscle and it can be strengthened like any muscle.. You need to do brain exercises..
anything that will stimulate new connections up there helps..
ie: while reading something, start reading backwards.. look at the page and try to read from right back to the left.
...if you are right handed, sit down and try to write a paragraph with your left hand..
...walk backwards in your house, the next time you get up to go to the kitchen, walk backwards there
...do newspaper puzzles, etc..
...count backwards by 3's, 5's, 2's etc
....say our alphabet backwards
...Dance.. (thinking about feet and hand movement)
...drink water, staying hydrated is good for any muscle health
...next time you are outside, look at nature differently than just runnign out, jumping in car... stare at the trees, look at the green, look at the clouds. think about their color etc..
...just do anything out of the normal that you normally do to give your mind a workout..
...get plenty of sleep at night also.. your brain muscle needs it sleep too

when u fisrt met someone and they say for ex. Hi, my name is andy, you can say hello andy nice to met you. when you talk to them contiune saying thier name. maybe a little notebook would help you. write doen info you think you might forget later. that helps me out alot.
