"Non life insurance" is a term used by India's Life Insurance Corporation. Every insurance that is not related to life insurance is referred to as "non life insurance". Examples are auto insurance, home insurance, health insruance, and business insurance.

There is no 'blank form' to use. Each insurer has thier own unique application. And 'non-life' is way too vague. You need to go to an insurer & tell them you need one. They are not available here.

Write to them they can send you sample applications for auto, medical, disability, home, renter etc whatever you desire from them,
