Most that I know of are non-profit.. I'm using one that charges a $20 fee per month, but the charges vary per company.

Consumer Credit Counseling Service charged me a very small amount. Not even $40.
The thing was out of the $80 payments I was sending them (to keep the debtor off my back) the debtor was getting about $55 and some payments were late causing my interest rate to go up.
Not a good experience. I decided to pick up the phone and call the debtor and renegotiate a payment plan.
The cool thing is.... I am sticking to it!

Most of them are free to the consumer. They are paid by the debt collectors.
