Is there a Fiber Optic Modem or Phone Protection device that has 5" of fiber optics between two PC boards with either screw terminals or RJ11 input or out put that eliminates ground Voltage differentials up to 75,000 volts? Something small, compact and not more then $400.00. If so, what is the web page?

Erm... it really isn't any longer a modem. That public sale is for a cable to apply to connect a modem for your computing device. especially, that is an RJ-eleven cable, usually wide-spread as a DSL cable. (also, DSL isn't an similar as fiber optic besides.)
