I am not an American, so I am not in the best position to give you the exact places. Assuming that I am the enemy who wish to drop nuclear bombs in the U.S., I wouldn't target large cities with civilian populations. Besides the human catastrophe it may cause, it really doesn't contribute any strategic advantage for me. So, my primary target would be the American nuclear facilities and nuclear missile sites. This would nullify America's chance for launching a retaliatory nuclear counter-strike. Furthermore, hitting such targets will double the effect of a nuclear chain reaction, because the nuclear explosions of my own missiles will add-up with the nuclear-explosions of the targets. About the locations of these nuclear installations, I don't know. But I guess there are lots of them in the Nevada and Arizona deserts. You wouldn't know the exact locations either, because they are highly top-secret and less-known to the public. My target priority would also be the American submarines and Aircraft-carriers, who are capable of launching medium-range nuclear missiles. My reasoning for this is the same as above. However, these military assets are continuously on the move in the vast oceans, so tracking them would be difficult.
Having accomplished this, it is then easier for me to strike at any remaining strategic targets within the U.S. mainland.

in an all-out nuclear war, then i'd expect any 'respectable' foe with a long-term strategy to strike first of all at important military bases, then at heavy industry and perhaps the seat of government. This causes the US to be defeated fairly easily- the military will be practically destroyed, and there'd be little opposition for any follow-up manuevors
If it wasn't a 'respectable' foe, then it's likely the most populated ares would be targeted, along with the seat of government. This puts public pressure on the government- and weaker leaders could be persuaded to surrender, or the country might capitulate to reduce casualties (bear in mind the loss of life would be tremendous)

Most likely the Northeast . They will make Denver the new capitol after Washington, New York ,and Boston are all wiped out . Ironically Bush, Cheney , Republicans, and many capitalists will be on vacation in Texas and will somehow avert this tragedy .

i live near the city of Elkhart INdiana, which has the 3rd largest rail yard this side of the mississippi. it was #12 on the russain nuk hit list, seriously. made me feal peachy when i found that out
