This is kind of a trick question. You probably didn't mean it as such, though. Human form is what you, hopefully, and I are. Jesus was born of man and of God. So technically speaking, Jesus hasn't appeared to anyone in human form.

This is a trick question, isn't it? He first appeared to his mother or the mid-wife if there was one. If you mean after the Resurrection, that was Mary Magdalena.
