So, my bibliography for the text version would look something like:

11(21), 945-968. Retrieved December 15, 2001, from

http://library.cqpress.com

I think the point is that the content of web pages changes so quickly that it doesn't mean much to give the electronic address (URL) without the date on which it contained the content you cited.

So "retrieval date" applies to the "electronic address" that follows it in your bibliography entry.
