A doctor goes to medical school and gets his doctorate, "MD", and does his internship, etc. Then he can choose to specialize in any field he wants to.
He can take more courses in his chosen field, and after further testing and training, be board-certified in his chosen specialty, but he's still an MD. He can be board certified in Family Medicine, also. More training means more knowledgeable doctor!
