There are three which come to mind, that cause many deaths. It would be difficult to name but one, for they are all deadly if not treated.
malarial parasite Plasmodium falciparum.
A tiny sand fly spreads one of the world's most deadly tropical diseases - Leishmaniasis.
The parasitic amoeba Entamoeba histolytica, a leading cause of severe diarrheal disease in developing countries.
Were I asked to decide which was the nastiest, I would choose Plasmodium falciparum. There are parasites that affect other hosts than human, but I would suppose you are referring to parasites that are nasty to humans, in your question.

Elephantiasis is a nasty disease caused by round worms in the tropics. It clogs up the lymphatic system and causes massive swelling. The bacteria causing MRSA , a staph infection resistant to most drugs can be quick and nasty and may take months to cure if you survive.
Lyme's disease and syphilis are easily cured if caught early, but are incurable and nasty in the later stages.

i'm not really sure what a parasite is, but maybe its that one flesh eating disease, that sounds scary.
