Formal:
¿Habla ingles?
(pronounced abla in-gles)

Informal:
¿Hablas ingles?
(pronounced ablas in-gles)

Plural:
¿Hablan ingles?
(pronounced ablan in-gles)

Generally the formal version is safer if you're not sure with to use, but in many place, particularly in much of Latin America, people primarly use the infomal version, even if they're talking to a complete stranger. You'd also want to use the informal version if you're talking to a child (who's significantly younger than you), or if you're still a kid, if you're talking to someone your own age. If you're a kid talking to an adult you've never met, you'd probably want to use the informal tense, but when I was in Mexico last summer I noted that even young kids talking to adults they'd never met used the informal tense. Either way, you're not going to offend anyone by using the wrong tense, especially since they'll know you're not a native speaker.

You'd use the plural tense if you were talking to more than one person. In Spain there's also an informal plural tense, but it's not used at all in most of Latin America, and since I don't remember the verb ending, I didn't list it. I'm having trouble imagining a senario you'd want to use the plural when asking someone if they speak English, but if for some reason you were asking it to a group of people, that would be the one to use.

¿Habla ingles Usted?

You would want to use this form (habla) instead of hablas because habla is formal and if you have to ask if the person speaks English or not, you don't know him/her well enough to use informal tense.

Note: The H is silent.

You should say "Habla usted ingles?" if you are talking to a stranger. You would only say "Hablas ingles?" if you already knew the person whom you were asking, and that is a rarity.

In spanish, to say "Do you speak english?"
would be:
¿Hablas inglés?
or if talking to a well-respected person:
¿Usted hablas inglés?
it is pronounced just how it looks
