Most mutations are bad. Statistically, it is nearly impossible for evolution to work via 'random mutation'. That's why Evolution without some form of Intelligent Designer is patently ridiculous. The odds are just so skewed (10^-29) it's not worth laughing at.

It can be either. It happens and if it's good the adaptations enhance the survival chances of a species however the great majority of species go extinct eventually.
