I think it is just what you prefer. I personally like to use the Mac 3. I joke and say I can intubate anyone with a Mac 3, even a baby. I like the Mac because it is a little wider then the Miller which makes it easier to get more of the tounge out of the way.

I prefer a Macintosh, but there are some situations in which the only blade that will work is a Miller. Just try your favorite blade to start, and if you need to, you can always switch to a smaller or larger size. Over time, you will figure out what you're comfortable with!

This varies depending on the situation. Is this for a full grown adult or a child? That makes a difference here.
Basically consider that the Macintosh is generally easier to use. The Miller blade does provide a much better view of the vocal cords however, which IMO is most important for placement.

The one YOU'RE most comfortable with.
When I trained, I saw anesthesiologists that would only use one or the other.
In my 15+ years in the field, I preferred Millers and was pretty successful at it.
Having said that, if there was a patient that I just couldn't get with a Miller, I'd switch to a Mac.

Fortunatly, I have only needed to intubate someone 3 times in my entire career. Each of those times I have used a MacIntosh. As people have said, its whatever you are most comfortable with and works better.
The very first time I had to intubate I used a Miller and don't plan on using them again. I just don't feel comfortable using them!

I prefer to choose my blade according to the patient. Ther size, long neck, short neck Etc. If you like a wider blade try the Grandview blade it is wider than a normal blade and it comes in adult and Pedi. sizes.

I prefer a Mac for general use, but a Miller for peds and trauma (c-spined). It really is whatever you get comfortable using.
