Tell her the truth, and offer your sympathies.

I guess everyone is different. Breaking bad news is something I have always been able to do. The straight story with compassion, no hemming and hawing.

I would have her sit down if possible."Maam, I am very sorry to have to tell you this, but your daughter has been involved in an accident and was killed." They know it's bad the second they see it's a cop on their doorstep and their kid is not there with them. Then once they get that part, they will want to know the details so you go there.

Generally you can use law and order type of lines as a starting point. Also make sure that you are at the right house. ("Mrs so and so I regret to inform you..........)Just remember two main suggestions though. The person should be sitting down when you tell them, and ask if anybody else is home, it generally helps to keep them in "check"

I'd say , Mrs.----- can I come inside and talk to you a minute...
I'd have her sit down and then I'd say theres been an accident and her daughter was involved. Then I'd tell her I was sorry, she didn't make it. If she gets too upset, call for an ambulance, help her call her husband, friends, family. Make sure she's calm and has help before you ( I ) leave...never leave family alone and upset, it makes them think you don't care..

Do you know why the child was out without supervision? If not, you would first ask about the child and if the parent knows where they are. You would want to ensure that there was no negligence. Then you would get the parents inside, ask them to sit and tell them in a straightforward manner with a respectful tone. Finally, you arrange for assistance through your agency's victims' assistance program.

Maam, there has been an accident. She will break down right there. Then be as gentle as possible with explaining the incident. Hugging might be involved for comfort.
