Bearing spacers are usually a tubular shaped device that keep bearing races apart and parallel. Modern bikes have this function built into the components.

A bearing cage holds the individual ball bearings inside a race. making it easier to replace .

An axle spacer fitted to your rear wheel is used to hold your rim central to the frame.

If your talking about those metal rings that the ball bearings are mounted in, they just make general maintenance easier as far as cleaning and re- lubing is concerned. They have no effect on performance.
