Ok this sentence doesn't have a correct structure.

¿Se supone que el amor duela tanto? = Is Love Really Suppose To Hurt This Bad?

I recomend you to translate complete ideas, instead of translating word by word.

It's a very bad translation to Spanish in the first place, probably using an online translator. I guess the original sentence in English was:
Is love supposed to hurt this bad?
In Spanish it should say:
¿Se supone que el amor duela tanto?

The correct sentence/question should be:

¿El amor se supone que duela tanto?
or
¿Se supone que el amor duela tanto?

Is love supposed to hurt so much?

they are trying to say " if love is suposed to hurt then its bad" the mispelled and the gramar is wrong
