Obtain forms from the IRS Web site www.irs.gov ,http://www.irs.gov/formspubs/article/0,,id=98339,00.html
Any refund due for tax years 2001 and 2002 have reached the statute of limitations on refunds. In other words you will not get any refund for those years. But if you owe for either of them you will be charged penalties and interest on what you owe.
You have until April 15th to claim a refund for the 2003 return that is the statute date for the 2003 return.

Just use the appropriate year's forms. Do it as if you are filing a regular tax return on time. However, you will need to add another form requesting an extension to file the return (even though after the fact) and include a check for the late fees.

The best thing to do would be to go to a tax professional like H & R Block because tax laws are so complicated and they can change from year to year. You wouldn't want to end up in hot water with the IRS.
