SR 22 is just a form for high risk drivers to prove they do have insurance in place before getting their license back. Contact your agent and he will handle all the details for you. Prepare to have a very high insurance bill for the next 3 years.

You must contact your auto insurance carrier for an SR-22. The insurance company will fill out the form and mail it to you. Then you must submit the form to the Department of Motor Vehicles in order to obtain a valid driver's license. An SR-22 form is basically for "at risk" drivers. Good luck!

Call your insurance agent. They will request one and the company will issue it and send it directly to the motor vehicle department so you can get your license reinstated. Some states will accept a faxed copy of the SR22 and others require the original. Check with your state and get a fax number if they accept a faxed copy and then call your agent with the fax number. Not all companies will do an SR22. You may need to switch insurance companies if yours will not do one.
