The question of why a cat will do this is not an easy one to answer. For this reason, finding a solution to the the problem can be a little tricky.

The reason that a cat might be scratching can range from simple to complex.

It may be that the cat had a bite (flea, spider or other) and began to scratch. It may have broken the skin and then because it continued to cause more pain and irritation the cat continued to scratch more and more and now you have a sore in that spot.

It may be that the cat was experiencing some sort of anxiety or stress and started to scratch as a nervous behavior which is not unusual for some animals. Maybe he took the hair off and then got down to the skin and then continued to scratch until there was the sore. And now because it continues to hurt he keeps scratching it.

It may be that there is something going on in the skin itself - some sort of nasty little bacteria or fungus that is causing irriration.

Or it may be that the cat has developed an allergy to something in the environment.

Figuring out what is going on can be a process of elimination and sometimes it is just impossible to tell. The one thing that is true though no matter what the cause is that once a cat gets an itch and breaks the skin - he is going to keep scratching unless you intervene. Once a cat has damaged his skin he won't stop on his own and things usually just keep getting worse until the skin gets infected. You really don't want that because then you are getting into more serious problems. The solution will definetely include a trip to the vet - probably an anti-inflammatory (steroidal) injection (this helps to calm him as well as cut the itch or pain), antibiotics (if the skin is infected) and maybe even a plastic e-collar around the head depending upon where the cat was scratching. If, after you do all of the above and he continues to scratch you may need to do a skin biopsy and or allergy testing. Good luck on this one.
