I'm not a doctor or anything, but it seems obvious you seem to have a problem feeling safe when you fall asleep. Feeling safe makes falling asleep much easier, obviously, and if/when you wake up at night makes falling back asleep easier.
You need to get that feeling of safety somehow. The easy thing to do would be to stop believing in ghosts, or at least the harmful types. If that's not possible, move to a different house. It's a bit extreme, but they should work.
Barring those, research your house's history. Almost every "Real Ghost Story" has some kind of emotional or traumatic event linked to it. If nothing like that has happened in your house odds are, even if you believe in ghosts, your house is ghost free. I've never heard that just because something is old it’s haunted.
After you get that feeling of safety turning off noise is the next step. If it is the sounds in the house that make you scared in the first place, try ear plugs so you hear nothing at all.
Other more mundane things you should in addition to the above:
Be caffeine free for a few days, go to bed earlier regardless if your tired or not, spend the hour before bedtime reading (A fun book that does not require thought and is not scary), and drink a warm glass of milk or chamomile tea about an hour before going to bed.

I would try just sleeping in like a t-shirt and some shorts or just a t-shirt. I CANNOT fall asleep without the noise of a fan or without it being pitch black. The nights that I still find it difficult I take 2 Benedryl and then I'm out. I believe there is life after death and anytime I scare myself by watching a movie like 'Pulse', I still close the door for the dark just not as much. Another thing you could try is going to bed with soft music in the back ground or an alarm clock that will play the sound of rain that can be soothing as well. I hope these tips help you some and happy sleeping!! : )

I often have the same problem. Wha really helps me is Zen meditation. It's really simple and you can start with doing just 10-15 minutes a session and build it up gradually. Sit in a lotus or half lotus on a soft cushion, spine straight and chin tucked in, your arm resting on your thighs and your hands together, with the fingers of the left hand on those of the right and thumbs touching lightly. Semi-close your eyes and try to only concentrate on your breath by observing it. Dont censor any thoughts; just observe them as they pass and gently tell your mind to come back to your breathing. You might find it difficult initially, but it will get easier with time.
Also, take a hot shower and drink warm milk with honey before you sleep.
Hope this helps you sleep better!

dont use the tv as your sourse of noise, get a c.d of soothing music that you can listen to quietly, this will help relax you... then try to clear your mind of all thoughts, difficult huh? not really... lay down and relax then just "look" at the darkness of having your eyes shut and concentrate on that i.e the nothingness of it... when thoughts pop into your head, just listen to them then say ok i have heard it and will deal with it later, go back to the darkness of your eyelids... sounds crazy dosnt it... but it really does work!!!

if you believe in ghost, they are not going to harm you .its my personal experience;once i got locked in a morgue and the ghosts didn't bother me coz I'm not afraid of them.if u feel they are haunting you then don't get bothered coz they are like bullies ;when you ignore them they don't bother you coz you've lost the interest in them and its not then a challenge for them to tease you.so,RELAX!hun.this is not a made up, I'm telling you the truth.
Slow music helps
take A warm glass of milk before sleeping.
pray to GOD for a good sound sleep.
GOD BLESS YOU.
HAVE A GOOD NIGHT SLEEP TODAY; YOU WILL I KNOW!!
