Everyone is different, but if you're having trouble concentrating or you're really restless, it's time for a break. You'll be better off in the long run.

Get plenty of sleep and don't try to pull an all-nighter.

In general it's best to study for one hour, then take a 15 minute break. But it varies from person to person.

that really depends on the person, and how you are feeling at the time, and how stressed you are feeling. I would say at least every two hours, any more then that could really stress you out, to the point that you are not going to retain what you are studing for the test. Even shorter periods may be good, like i said it would really depend on the person, and how much it was stressing them in the first place to take the test.

But it takes about 45 minutes for your mind to exhaust from studying and then you require a 15 minute break.

One hour, break, then one hour. The mind cant absorb everything if you go on nonstop.
