Can I use my last name as my childs last name instead of his fathers as we are not married? Is this ok to put on the birth certificate.

You must have moved after your child was born--as normally, they mail the original birth certificate to the address you gave the hospital.

If you are not married to the dad, the baby can certainly have your last name. For that matter, they would not put the father's last name down, unless he signed a paternity paper.

Yes, you can leave father's name blank if you want to, and give the child your last name. Or, you can get the father to sign paternity papers, so his name will appear as the father--but still give the baby your last name.

I'm a little confused. Where I live you fill out this information before leaving the hospital. You don't get an actual copy of the birth certificate unless you go to the Office of Deeds or your areas equivalent but the information is already recorded. But if that's not the case in your area then I'll answer the actual question. It's completely up to you with pro's and con's to either side. If the father might leave you or stop supporting the child (horrible to think about at the moment but it's a possibility to consider) then giving the child their last name and having them sign as the father will go a long way in terms of establishing paternity in a court should you have to file for child support. However I think that just having them sign as the father goes far enough if you want to give your child your last name. Another thing to consider is do you think the father will stick around, even to the point that you get married? We gave our daughter her father's last name because we planned on getting married eventually...and figured it would be confusing if she didn't have the same last name as we did and a hassle to change it. Do a little soul searching and discuss it with the father. It's a very personal decision.

You were givin paper work in the hospital for the baby as what name you wanted and what last name you wanted. That will be the name on the birth certificate that you will get in the mail once you send for it.

You will always have the option to change the first name and/or last name at anytime until that child is 18. It will get harder to do the older the child gets cause of them knowing themselves by one name and you wanting to change it.

If the father is one the birth certificate you might need his signiture to change the name of the child. Last name and/or first name.

There is a birth certificate, you just don't have a copy of it. (unless you gave birth at home....) In our state, they don't mail you a copy unless you request (and pay) for one.

In the hospital you should have filled out paperwork which is sent in to the state. From there sometimes you have to request a copy.

Look up your state's department of records, then contact them for a copy of the b.c. There should be applications on-line.

If you don't like the name as shown on it, you can request a change. Each state is different on the laws how to do that.

You have to use the last name you put on the paperwork in the hospital. I wasnt married to my sons dad... therefore I gave my child MY lastname. If ever you two file for paternity they'll give you the opition of a name change from yours to dads.

Spain ruled the land referred to now as Arizona till Mexico performed its independence in 1821. The U.S. gained this land interior the Mexican-American warfare in 1847. the unique citizens are interior sight individuals and/or Hispanics who have been as quickly as voters of Spain and later of Mexico. the recent Arizona immigration regulation makes it a state crime — a misdemeanor — to no longer carry immigration papers. Does this propose that everyone people of eu extraction who have been for sure immigrants in some unspecified time interior the destiny might desire to hold their immigration papers? somewhat the Arizona government does not desire to annoy the "genuine" Arizonan voters whose ancestors have lived interior the state by using fact that earlier it replaced into even a state.
