"Hang-up" is when is when you are talking to someone on the phone and you put the receiver on the hook or turn the phone off.

As a slang term, it means a problem someone has. They get stuck in their way of thinking concerning the problem, hung up on it like a coat on a coat hook, not being able to get loose from it or past it.

It means both things. It can be a problem, as in a personal issue that you have trouble getting over ("I am hung up on my ex-boyfriend; I can't get over him."), or a problematic situation ("I am hung up at work and can't make it to dinner tonight.")
It also means the act of ending a telephone call: "To hang up on someone."

hang up as a phrasal verb means to end a telephone call (I'm going to hang up on you now")

hang-up as a noun is a problem or a concern ("He spoke to his shrink about his hang up -- he can't stand girls who wear heels")
