Best bet would be laser surgery clinic connected with a large univeristy hospital. They would have the best and newest techniques and equipment, and the cost would be more affordable than at a private hospital. This is a consideration as insurance probalby won't cover it.
Not all tubal ligations can be reversed, it depends on how neatly the original job was done.
When I put "reverse tubal ligation" into the search window I turned up a lot of links, all over the USA and all over the world. So put it in your search window, and look at the various websites, find someone you can work with, preferably in your area so you won't have the additional expense and stress of a lot of travel.
