I thought it was diarrhea and other complications from unclean water, or maybe TB. I know in the U.S. heart disease is the top killer, but what about worldwide.

Heart disease and stroke worldwide as well, at least in 2001...

You were right. According to the Collier County Health Department in Florida it is diarrhea. You are also right on the cause. They drink the water and an organism makes them sick. They get diarrhea and then die from the effects of dehydration.
