I would contact the cruise line you are going on. Generally for a 5 night cruise there will only be one formal night. However, each line may be a little different. Some have more, some have less, some are really strict while others will let you where regular dress clothes for formal nights (as long as it is nice but no shorts, short sleeves, etc)
Also, there may be different events where you may want to have something formal, i.e. if you have a lunch with the captain. Some lines offer that and the attire is usually formal. Your cruise line and itinerary would really help you figure this out. If you call up the line, or ask your travel agent they will get you the correct answer for your actual cruise and let you know exactly what is expected.

I have been on a few five night cruise with Royal Carribean and both times there has only been one (1) formal night. I have another planned in August and I asked my TA and she confirmed that there will only be one (1) on this as well. So my bet is buy one formal dress and then as many fun "summer" dress as you need for the rest of the times you will be in the dinning room. Have a blast and take care!

What cruise line? Your answer varies depending on the line you are sailing on.
If you're sailing on Norwegian or NCL America the answer is 0, however in general figure two to three. If you booked with a travel agent call them and ask them and they should be able to tell you, and if they don't know they will find out.
Depending on the line you might also be able to find out for sure on the website.

How "formal" depends upon the cruise line you are taking.
On the Carnival cruises we have been on, we have seen from very formal cocktail/evening dresses and tuxes to more casual summer dresses.

I know for a fact that there is One formal night with a tux and 1 business casual night the rest are just skirts and sundresses and kakki and polo's for guys. NO shorts are allowed at any time in the dining room!

There will only be one really fancy dinner, to the other meals you can just wear skirts, dress pants etc.
I am positive it is one, not three or four, those people have no idea what they are talking about.
