Oh that's rude I think. You made a connection (a play on words) between the name of a town and the name of a mans downstairs parts. This could even have been a Joke in other circumstances. However I think that I will answer the question on face value.
I think when you fell you should have apologised for being so familiar even though it wasn't your fault, and not tried to change the subject by asking about the destination details.
Etiquette wise it's always better to think of the other persons well being first before your own. And it may even have been a good idea to buy the person a cup of tea and collected a warm blanket and wrapped them up in it in case of trauma.
