I haven't had one in a while, and don't even know where to go. Family doctor, maybe?

Go to a general practitioner (GP), aka a "family doctor". All this means is this type of physician isn't a specialist, although a specialist is qualified to conduct a physical checkup as well. For that matter, I played high school football with a guy who got his physical from a vet, so I guess they're qualified as well.

Any doctor licensed as a GP (general practitioner) or Family Doctor as others call it should be able to provide a physical.

You would just go to a normal GP / Family doctor.

Regular General Practioner. He does the routine blood work and if anything is a little abnormal he sends you to a specialist.
