I have to file this year to get credit for my 2006 taxes. I am from Illinois and worked in Illinois in 2004.

Call your employer and ask for a replacement copy.

They should be able to do this easily. I work in payroll and we are asked at least once a year for replacement copies; it's not a complicated process, so if they give you grief, ask to speak to someone at a higher level.

Go to your local IRS office and they can run a print out of all your income that was reported for 2004.

1) ask your old boss and see if you can print one up

2) call IRS for a free transcript at 1-800-829-1040
