Because medicine absorbs faster on an empty stomach. But if this causes an upset stomach, eat something.
And yes, some foods are worse than others. Foods that you know will...uh...run right through you will take the medicine with them when they go.

Taking food with penicillin may make it less effective and it absorbs better on an empty stomach. I could not find any specific foods that would make it less effective, however, it did say that if penicillin upsets your stomach you may want to take it with food.
There are some drug interactions as well.

we can, it just makes the penicillin less effective as its broken down with our food and not dissolved into the blood as easily.
