the reason why i am asking this question is that my father had stroke 1 year back and doctor advised him to take warfarin depending upon the INR test. but some other doctor advised that more than 5 mg of dose should not be taken. however he is on 5mg warfarin but his INR test always show INR between 1.5 to 1.8 only. i am worried because different doc have different opinion. can somebody help me???????????

Arshad261, I'm sorry that your dad had to suffer such an illness. It must be really tough.

I'm 26 and I've been on coumadin for nearly 3 years. I'll be on it life-long because of a severe clotting disorder I have. My dose fluctuates anywhere between 7.5 -- 12.5 mg. depending on my INR results.

Warfarin, as you know, is helping to keep your dad from suffering another clot. The INR test that he has done measure's how effectively the warfarin is "unclotting" his blood. If his INR is too low, it means that there is not enough medicine to do the job. If it is too high, it means that his blood is not clotting fast enough in a safe range. This is why his dose of warfarin may and will fluctuate. It's all about the test.

I really have to disagree with the doctor who said he shouldn't go higher than 5mg. If his body needs more (as is told by a low INR), then he is risking a second clot by not increasing the dosage.

Do you know what your dad's INR range is? Mine is 2.5--3.5 I have a home monitor where I can test at home now. It may be something you want to consider, if the insurance will pay for it. Here's a website for one of the monitors out there www.hemosense.com

Oh, I almost forgot to ask...is your dad on any other "blood thinners" besides Warfarin? Warfarin helps prevent clots in the vein, but not necessarily clots in the arteries. You might want to consider asking his doctor about baby asprin or Plavix in addition to Warfarin therapy.

We have had people take up to 8mg of warfarin daily at the nursing home I work at. It does all depepnd on your INR results as to what dose the doc will prescribe. Some of our patients take one dose 2-3 days a week and another dose on the other days of the week, It depends on the doc and the INR results.

It is very important to check the INR level while one taking warfarin (coumadin), in this case best suggestion is, find a good internist/neurologist (if you don't like the doctor who advised you to take warfarin depending on INR level) and stick to that particular doctor, if you think you have problem finding a good doctor, best thing to do is, make appointment with any University Hospital's Stroke Physicians, usually university hospital physicians are the best, cause they are very up-to-date with advanced treatment. I hope your father will be alright soon!!

I work at a coumadin clinic, and we have patients that weekly dose ranges anywhere from 7mg a week to 125mg a week.

The reason being, warfarin is digested and broken down in the liver. If one person has a very strong liver, or a very damaged liver, it will really affect the way your body metabolized the pill.

The dose should be determined by his INR. IF he is continually low, they need to adjust the dose. 5mg is usually the dose they start patients out on (the average dose) to see how it affects them.
