Foolishness & wisdom are two ends of knowledge. A fool is at the lowest rung & does not have the taste of wisdom, therefore he is foolish enough to mistake himself for a wise person.
However, the wise man is at the other end & has seen the entire gamut of all the stages from foolishness upwards, therefore he is in a better position to know the distinction between a fool & a wise man.
Since there are various stages and dimensions of knowledge and the act of acquiring knowledge is dynamic, not static, therefore a wise man who has reached the end of one stage has knowledge of the fact that he is at the lowest rung of the next stratum of wisdom, therefore a 'fool' vis-a-vis the next stratum.

The person who actually is a fool, believes that he's actually a wise man. But a truly wise person knows that he will always be foolish.

it is taken to meaning that anyone who is silly enough to think of himself as all knowing or wise is generally ignorant of a good amount of information out there. A wiser person will not make the same mistake by assuming he knows everything. He is more likely to admit his faults and his ignorance and so is a lesser fool.
