It really depends upon your taste of cheeses. I make mine with Provolone and Swiss. You could also go for a Blue Cheese or Goat Cheese. Others say American Cheese is good and Velveeta works too. Have a Cheese Steak buffet night and top with a variety of cheeses.
I have even done a Pepper Jack Cheese with my Cheese Steak.
I don't think there is a wrong answer :-)

It depends on the region and the establishment.

Some popular choices are:
1. white American cheese (my favorite cheesesteak shop uses this kind)
2. yellow American cheese spread
3. yellow American cheese
4. Provolone cheese

Melted Velveeta - you spoon it over the steak and toppings.
Messy, but good!
