Optical or voice command would be optimal, but not practical. I would have to say the answer is disposable sterile covers for keyboards and other input devices.

I would suggest a foot pedal. Something made of stainless that could be autoclaved would be good.

Another possibility would be a "bite trigger" or "puff trigger" similar to what paraplegics use. Just keep it in your mouth behind your mask and blow or bite to input.
