I would say the Sanctuary Lamp burning in the area where the tabernacle is kept, whether at the High Altar or a side Blessed Sacrament chapel.

There is a candle somewhere near the tabernacle that is always lit when the Blessed Sacrament is reserved there. (Well, okay, it's obviously not lit when someone is replacing the candle itself, but you know what I mean.)

For the most part, the only time a tabernacle is empty is the time between the end of Holy Thursday night mass (Mass of the Lord's Supper) and the communiont rite at Easter Vigil (Holy Saturday night) or if the church is somehow in a state where it has been desecrated (e.g. fire or extreme vandalism).

No sign is needed, because it always is. At least in a Catholic or Orthodox church.
