The only real difference is the amount of milk froth on top.

Cappuccino - 1/3 espresso, 1/3 steamed milk, and 1/3 milk froth
Latte - 1/3 espresso, 2/3 steamed milk, and dollop of milk froth

latte is mostly milk and little foam
cappucino is more foam and little milk

usually 1 shot of espresso for every 8oz or 1 cup of steamed milk (temp at least 150degrees celsius)
add vanilla/almond extract
or chocolate syrup

the first guy has it right. don't listen to the guy from starbucks. I could tell he was a starbuck barista when he said to put in almond extract. Starbucks Sucks!
