One problem could be no or low airflow from your defroster ducts. Check them for obstructions. You could also have airflow problems related to the heater operation, a door or flap not opening or closing somewhere in the system.
If the windows are fogging up on the inside, this might be a symptom of a leaky heater core. Check for damp carpeting and an antifreeze odor in the car. Running the AC with the controls in defrost mode is an excellent way to clear foggy windows. Some car models activate the AC automatically when you switch to defrost.

A quick remedy is to wipe your windshield with a product like Rain-X, that will keep it clear, but you will have to do it every once in a while, because it wears off. Your defroster should clear the windshield, because while on defrost, the a/c compressor is running to dry the air. Get the a/c checked to make sure it still has refridgerant in it, and also get your heater core checked, because if that is leaking, it will fog your windows pretty bad.

Usually the Air Conditioning removes moisture from inside the cabin. You may have a vacuum leak that is not allowing the duct doors to move. If the A/C has lost it's charge, the compressor will not come on. In other words, the A/C is not working at all.
You may have a heater core leaking and antifreeze is spraying onto your windshield.
Your bet bet is to go to an ASE repair shop and have them perform a couple simple tests to determine if it is A/C or vacuum or antifreeze. Do not be afraid to call around for pricing BEFORE you drive to the shop. Just ask, "How much to check it out".

Use fresh air if the car has that option, if the fog on the windshield is oily and hard to wipe off your heater core might be leaking, make sure your cabin fresh air filter is clean if the car has that option. Every car should have a way to let fresh air in as well as stale air out, if either is plugged it could cause this problem.
