I don't know about a dealer, but look in your Yellow Pages (Or White Pages) for any local DURABLE MEDICAL EQUIPMENT company(ies) and approach them with this.

Some of these companies may be interested to purchase your van to use it as patient-transportation services.

Contact The United Way. Salvation Army or whatever good Charity that you prefer. It can be used as a tax write off but more important someone that needs it will get it.
