Actually I just had some clients return from Tibet back to the US recently. They spent about 10 days in Tibet visiting Lhasa/ Gyantze / Xigaze, which is a great triangle to cover the highlights of Tibet.
There is train now that can take you directly to Lhasa from Beijing, which takes 48 hours. This rail service started in July 2006. However because of the harsh Winter weather, there is no train in the Winter.
We arranged for our clients the whole package tour, including the flight (They chose to fly in and out of Tibet instead of the train), the Travel Permit to Tibet, the hotel accommodation, meals and all the sightseeing with private tour guide. However, we can help you to purchase the train ticket to Tibet.
The scenery in Tibet is absolutely extraordinary! I talked to my clients both when they were in Tibet and after and they said they didn't feel the altitude sickness very much, other than when you walk upstairs you feel a little bit labored breathing.
The best time to go to Tibet, of course it's in the Summer, however the rate might be better in the Spring and Autumn. As you probably have heard that it's a little bit harsh in the Winter, but if you are going to Lhasa only, then it's open all year round.

Hey...well, it's pretty easy getting into China..you can go to www.visarite.com . They will get you a visa, no matter where you are in the U.S. They are very reputable, just mail them your passport and other needed documents. You will either get a 1 or 3 month traveller visa.
As for Tibet, you need a special permit to go there. Perhaps visa rite can help you with that too. There is a train going from Beijing to Tibet.
For Nepal, I believe you just need a regular visa. Go to Nepal's embassy website in the U.S. to get more info on that. Be careful going to Nepal, though. They are having political problems now, and it could be dangerous. Do Not travel there alone as a woman.

First tip: When you are in China, don't tell people you are there to visit China and Tibet... Politically incorrect.
2nd tip: Get some shots before going.
3rd tip: Respect their cultures even if you feel what you see and hear are somewhat shocking to you. Don't forget, that's how they have been for 1000s of years.
4th tip: (somewhat related to the 3rd tip) open yourself up, your mind, your heart, then you will enjoy a lot more than as a pass-by-visitor.
