Make sure that she is given any medical cards/coupons. Write & sign a letter stating that "_______________(Your mom's name), has my permission to authorize medical treatment for __________________________(Your son's name). Sign & date it. They are good for 6 months, so if she watches him while you work and he has an accident and you cannot get their in time for treatment she can sign for it. I do this when my 2 year-old stays with my parents. You do NOT need to have anything notorized, or have a Power of Attorney for this. Just the letter like I wrote out above and your signature and the date are sufficiant.

I am sure all will be fine with leaving your son with your Mother, but you should plan for the "just in case". Your Mom should have his doctor's number, local hospital, etc....
Make sure she has all your numbers.
If you are worried about the boy's father causing trouble then I would make sure you have all your legal documents, you should anyways.
You could draw a document up yourself, have it signed my you and your Mother and get your lawyer to sign. Or get your lawyer to draft something up.
First time away is always the hardest. It will be harder on you then your son or Mother, believe me, I know. I am a Mom of 3.
Try and have a good time on your trip.

Hi, you are worrying yourself sick. If your son is doing well so far, what is likely to happen to him while you were away for only 4 days?
If he is sick, your mom can bring him to your regular paedictrician with his documents/papers.
Go on, have a break. You deserved it. As long as you can be contacted via mobile overseas, she can reach you or someone close, your son will be fine. Relax. Good luck!

You should write up paperwork stating that you give your mom full rights to have you son treated in any case while you are gone. all so paper work stating that you are leaving her in charge of your son. you should all so leave copies of his insurance card for your mom and his S.S. number just in case there were any medical emergency's and leave a number where you can be reached

Just a power of attorney, i believe should work. make sure its notarized. Just basically given her permission to take him to the doc, if necessary.
