Does anyone know if they actually do this, and if so how much of a hassle is it?

I have 2 friends on SSI/SSD. Yes, they do reevaluate your case periodically.

Initlal approval for payments is based on your inability to perform in your regular career or occupation.

As time progresses, you are evaluated on your ability to perform in any activity for pay or compensation. The thinking is that if you can be retrained and rejoin the workforce in some capacity where you would earn at least as much as your SSI/SSD payments then you should do so. They will provide assistance for school and reentry into the workforce.

I know one person who went that route and is now off disability and is making more money than they did before the original disability. She's still physically handicapped, but is anything but economically handicapped.

Dealing with any bureaucracy is a hassle, and SSA is no exception.

Federal bureaucracies have their mandatory guidelines. Re-evaluating is one of those guidelines.
A relative of mine had to have a kidney transplant. He was on disability for five years. He was re-evaluated and cut off after five years.
I worked with a woman who had a kidney transplant. She returned to work as soon as the doctor permitted it, approx two months after surgery.
What is disability to one may not be disability to another.

Yes,you are evaluated every so many years. It really isn't too much of a big deal, they contact your doctor who has to verify that you are still disabled and unable to return to work.If there is a problem with what your doctor says, they may send you to one of their doctors for re evaluation.I have never had a problem.
